How to Extend a Custom Health Journal

Care3 offers members curated Health Journals with professionally-reviewed content to address a selected issue. Custom Health Feeds are an easy way to collect data, see trends to address a particular issue of interest. You may want to extend a Custom Health Feed longer than its original length.

Extending a custom health feed is easy. However, you must complete these steps from a computer and not a mobile device. Follow these steps:

1. From your computer, log into the website at www.care3.co. Click the “Log In” button in the upper right corner. Log in using your username and password.

2. When you get to the Home screen, tap the Health Journals icon on the left side navigation bar.

 

3. On the Health Journals page, you will see your customized feed in the list. To discontinue, click the pencil icon next to the end date.

4. Change the “End Date” to your desired “End Date”. It must be within one year. Then click the “save” button.

That’s it! Your customized feed will continue sending your periodic items.

How to Discontinue a Custom Health Journal

Care3 offers members curated Health Journals with professionally-reviewed content to address a selected issue. Custom Health Feeds are an easy way to collect data, see trends to address a particular issue of interest. Members may want to discontinue a Custom Health Feed before it runs its course.

Discontinuing a custom health feed is easy. However, you must complete these steps from a computer and not a mobile device. Follow these steps:

1. From your computer, log into the website at www.care3.co. Click the “Log In” button in the upper right corner. Log in using your username and password.

2. When you get to the Home screen, tap the Health Journals icon on the left side navigation bar.

 

3. On the Health Journals page, you will see your customized feed in the list. To discontinue, click the pencil icon next to the end date.

4. Click the “end now” button. Then click the “save” button.

Your customized feed will stop sending your periodic items. Your Custom Health Feed will remain accessible from your Home screen and all data you’ve entered up until you discontinued is saved and preserved.

Create a New Health Journal Timeline

You can create a new Health Timeline for another person to keep all information organized. You can create your new Timeline via the Care3 website or Care3 mobile app. Instructions for both methods follow.

To create a new Health Journal Timeline in the mobile app, simply follow these steps:

1. From the Home screen, press the “Chat” icon. We use a chat icon because all Timelines also have full secure messaging capabilities.

2. By tapping the “+” button to Invite to Timeline (it may say “Invite to Health Chat”), choose “Email or Phone Number” from the menu. Then add YOUR EMAIL (use the email address you used to sign up).

Note: Tap “+” again to invite anyone else to the new Health Timeline. You can add others later, too.

3. Enter your Health Chat/Timeline Title. It should be something like “Janet’s Health Timeline” to identify who the Timeline is for.

Note: Adding an Image is optional. It’s a nice touch, however, if you are creating a Timeline for your daughter to have a picture of your daughter.

Note: Select “Care3” for Organization if required.

4. Tap “done” in the upper right corner. Your new Health Timeline will be created and appear on your Home screen.

Any people you added will be invited to the Care3 app automatically if they aren’t members. If they are, the new Timeline will be added to their Home screen immediately.


That’s it! Send a message to say hello to anyone you’ve invited to the new Health Timeline.

 

Create a New Health Journal Timeline via Care3 Website: First log in at www.care3.co

The process to add a new health journal from the Care3 website is similar to the app.


Click the “Login” button in the upper right corner. On the next screen, use your username and password to log in.

1. Tap the “chat/journal/timeline” icon in the upper right corner of the left panel on the Home screen. A popup menu appears in the left panel.

 

2. By tapping the “+” button to Add to Chat/Journal, add yourself (enter your email address). Repeat the process to add anyone else you want to have access. You can add others via email address, phone number, or choose from your Contacts.

2a. Enter your Health Chat (Timeline) Title. It should be something like “Janet’s Health Timeline” to identify who the Timeline is for when you’re adding health files and information.

Get Started! Your Health Timeline

Do you have a binder with your past health history that you carry with you to appointments? Now you can transfer that paper binder to your Care3 Health Timeline/Journal that goes with you everywhere. You can add files including pdfs and media, track symptoms and events with charts, add appointments with notes, and share the entire set or any individual element with others whenever you want.

This guide shows you how to:

  1. Find Your Health Journal (on the Care3 mobile app and Care3 website)

  2. Add Files and Media to Your Health Journal (app and website)

  3. Invite Others to Your Health Timeline/Journal

  1. Find Your Health Timeline/Journal

Care3 Mobile App (iOS or Android):

On the Home screen, find the item titled, “(Your First Name’)s Health (Journal).” Tap into it and you will see a blank space. This journal includes ONLY YOU. When you’re ready, you can invite others.

Go to www.care3.co and log in using your mobile app credentials (email and password).

You will see your timeline in the left panel of the Home screen right after you view the helpful starter video.

2. Add Files to Your Health Timeline/Journal

Care3 Mobile App (iOS or Android):

To get started, tap into your Health Timeline/Journal (or one you’ve created for your Loved One).

1 .  Tap the “+” button in the lower left corner to show the media menu.

2.  Tap the camera icon to add a photo or take a new photo. Tap “Choose Existing” to select an image from your Camera Roll or “Capture Image” to take a new picture.   

Note: You can use the same steps to add video or audio files to your Health Journal. Simply follow the prompts after selecting your chosen media type.

3. Once you’ve added or taken the image, it appears in the message box. Type a description of the image. Use keywords like “labs” that will make it easy to search for the image later when you need it. Tap the “send” button (airplane).

4. View your item in your Health Journal. The image is saved on our HIPAA-compliant cloud server.

Care3 Website (https://live.care3.co):

1. Go to https://live.care3.co and log in using your mobile app credentials (email and password).

2. Click (select) your Health Journal or the one you set up for your Loved One.

3. In the right section, tap the “+” icon to reveal “add media” menu. Select “file upload”.

4. Then select the file you wish to add to your Health Journal and click the “Open” button.

IMPORTANT: In the dialog box that appears, write a description of the file. Adding a description makes it easy to search and find later.

Click “Open” to add the file to your PHI Health Journal.

4a. You can also drag and drop files directly from your computer’s directory into the Health Journal.

REPEAT IMPORTANT: In the dialog box that appears, write a description of the file. Adding a description makes it easy to search and find later.

That’s it! Repeat these steps for every item you want to have with you everywhere you go. With Care3, you now have access to your personal health information from anywhere at anytime.

3. Invite Others to Your Health Timeline/Journal

1 .  To invite someone (family, friend, doctor, nurse, etc.) to your PHI Health Journal, tap the “members” icon in the upper right corner.

2 .  Tap the “add member” icon in the lower left corner. It’s the one with the “+” sign.

3 .  Tap “Add to Health Journal” and select from the menu how you’d like to add the member. Most people choose “Add E-mail of Phone Number”.

4 .  Add the person’s email address OR phone number—or select from your Contacts—or select from an existing Care Team. Make sure “History Access” is green so you enable full sharing. This means you give access to all files and information shared in your PHI Health Journal to the person you invite.

5 .  Tap “save” and an invitation is automatically sent!

 

Now you have invited someone to your PHI Health Journal! You can also send secure text and media messages about your healthcare to the person you invited after they join. Take care and good luck!

Create a New Smart Health Journal for a Loved One

The “Smart” Health Journal makes it easy to see what is happening with your loved one and share that with others who may not be around each day. Smart Health Journals automate symptoms, vitals and task tracking by sequencing what is tracked each day in the mobile app. We have laid out the steps to walk you through setup of automated Symptoms tracking for your Smart Health Journal.

Note: Use your computer to create your Smart Journal, not your mobile device.

Note: Creating a new Smart Journal takes 10-20 minutes. Take your time! It will save you hours later.

First: Log in to Website

Go to www.care3.co. Click the Login button in the upper right to go to the login page. Enter the same email and password you used to sign up.

Next: Start New Journal

Click the “heart page” icon on the left side navigation bar (below the calendar icon). Your Smart Journal has three pages: Setup, Actions, and Review.

Click the “New Journal” button to go to Setup.

Setup: Add Patient Info and Journal Details

First and Last Name: Enter your loved one’s first and last name. (Note, if this Smart Journal is for you, add your name.)

Preferred Method of Contact: In the Preferred Contact Method field, add YOUR email if you are the patient or if you will be tracking on behalf of your loved one.

 

Add any Family/Friends if they will be tracking symptoms and vitals. If not, you can skip. You can also add other family/friends later if necessary.

Journal Title: Add the Journal Title (e.g., Jane’s Health Journal). This Title will appear in the mobile app and website Home and screens indicating where you you can track and add to your journal.

Time Zone: Choose the time zone when the journal reminders will be sent. This may be different from your current time zone.

Start and End Dates: Add the Start and End Dates. We recommend three months in duration. This will give you enough time to get really good data and charts. Note, Smart Journal durations must be shorter than one year.

You don’t have to add Family Caregiver or Family/Friend now. You can always do it later.

Click “save” to go to Interventions.

Problem and Objectives: Choose the main Problem (diagnosis). You can only choose one. Add any other diagnoses in the Objectives field along with what you want to achieve by tracking these symptoms and vitals.

Professional Team: Only complete this section if you are a care professional or if you have permission to add care professionals to your Journal.

Once you’ve entered all of the Setup information, click “save” to go to the Actions screen.

Actions: Add Symptoms and Vitals to Track

On the Actions/Interventions page, scroll down past the header information summary from the Setup page to add your Symptoms.

Symptoms: Click “add” in each group to see what Symptoms and Vitals are available. There are several groups so make sure you review each one to make sure you are tracking exactly what you want.

Start - End Dates: These dates are for the individual activity. The default is the duration of the journal. If you want to keep those dates, simply leave them. You can NOT choose dates outside of the Journal date range.

Frequency: Select how often you want to complete this action. Note: If you choose more frequent than once per day, you can also add the exact time of day you want the tracking Reminder to be sent.

Outcomes/Notes: Enter any helpful notes.

 
Don’t forget to click the “save” button on each symptom.

Repeat this process for each symptom/vital you choose. If you want to add items from any of the other Groups, you can repeat these steps as well.

When you’ve added all of the Symptoms and Vitals you want to track, click “review plan” to go to Review.

Review: Review Your Journal Tracking

Review: In Review, you can view all symptoms and frequencies. If you need to edit any of the symptoms, click “Actions” in the upper right to take you back to the Actions page. There you can edit any symptom.

That’s it! Your Smart Health Journal is ready. Click the Care3 icon in the upper left navigation bar to return to the home screen to see your Smart Journal.

Note: You will start receiving your tracking Reminders in the mobile app the next day.

Add Appointments to your in-app Calendar

Do you want to have all of your doctor appointments in an app and then have it remind you that you have that appointment? With Care3, you can. Here’s how:

An appointment is created as an “action message” in Care3. Click the “+” icon next to the message box in your conversation to open the menu. Select the “chat star” action message icon to add an appointment.

Complete the fields with all pertinent appointment details:

  • Appointment Title/Name of Doctor/Therapist

  • Date/Time/Location

  • Ride or Attendant Needed check box

  • Any Notes

When all fields are added, tap “save”.

Tap “x” in the upper left corner to return to the conversation.

You’ll see your new appointment in an action message with the red “accept” button.

Tap the red “accept” button to add the appointment to your in-app calendar.

 



To view your My Actions Calendar, first tap the “x” icon to go to the Home screen.



Then tap on the calendar icon in the bottom navigation (2nd button from left as shown here).

 


Tap on the date you wish to see the appointments for. Dates with appointments are indicated with a red dot underneath.





Below the calendar, the latest appointments are listed in chronological order.  Simply tap on an appointment to reveal the details. Tap on an action button to let everyone know the task status (accepted/done).

Once you’ve had the appointment, let everyone know by tapping that “done” button. The button color and adds a check mark to indicate completion. Well done!

*To add any extra notes about the appointment, tap "edit" in the upper right corner. Tap “save” after you’ve entered the notes.

Back in the conversation, everyone in your conversation can see the appointment happened and the notes. They now know what happened in your appointment.

So simple, right?

Care3 helps you manage appointments AND keep the people who care about you informed at the same time. Now add all of them so you have a full calendar of your healthcare appointments.

Incident Reports: Track Symptoms and Events

Tracking symptoms and reporting events/incidents are two immensely important activities in helping your doctors, nurses, and therapists provide you the best healthcare. An Event/Symptom Report allows you to manually track a symptom you’re experiencing over time (e.g., pain, nausea) and/or report a specific event (e.g., seizure, rash, fall). Make sure to create an Event/Symptom Report each time you experience a symptom or event.

1.  From within a health journal, tap the “+” icon next to reveal the action menu.

2.  Select “incident report” or “event/symptom” by tapping the icon.

3.  Complete the fields (image shows completed sections). Try to give as much detail as possible to help everyone understand what happened.

4.  Tap the “check mark” in the upper right corner to complete your report.

5.  To view the completed report, simply tap on the Action Message to reveal the details. Repeat these steps to add another instance of the same symptom or event.

To view a chart of your symptom or incidents, tap “View History” or “View Chart” to see real-time longitudinal updated charts right on your device.

View Charts of Your Tracked Symptoms and Incidents

One of the most important features of symptom and events tracking is to see the changes over time. You can do that easily in the app by tapping “View History” on the symptom and/or vital you want to view.

1.  To view a chart of your incident/symptom, in the bottom right corner tap on the “view history” or “view chart” text.

Start a New Health Chat

Texting has become the main way people connect using mobile devices. It's quick, not as intrusive as a phone call, and shorter form than email. People typically respond faster to texts than other forms of communication as well. More importantly, when having text messaging conversations about your healthcare with family or doctors, you want to be absolutely sure that conversation is private, secure, and confidential. iMessage, SMS, WhatsApp and Facebook Messenger are popular, but NONE meet government security standards for healthcare text conversations. Care3 not only meets these standards, but exceeds them for secure for healthcare conversations.

To start a health chat on Care3 simply follow these steps:
