Care3 is offering its world-class healthcare mobile messaging app FREE for 6 months exclusively for North Carolina PACE Association members.

Offer Terms

The NC PACE & Care3 partnership offer period is for 6 months or up until March 31, 2019

  • NC PACE members will receive FREE access to use the Care3 mobile app for up to 6 calendar months

  • At the end of the offer period, Care3 will charge $10 per program Participant per month for use of the Care3 mobile app

  • The PACE organization may have unlimited staff and family members on the app at no extra cost

  • The Participant total used for invoicing will be based on the total participants reported to CMS for that program in the most recent reporting period

  • If, at the end of 6 months, the PACE organization wishes to discontinue use of the Care3 mobile app, Care3 will securely transfer all data generated during the 6-month period at no cost to the PACE organization via encrypted CD.

  • If the PACE organization wants to move data from Care3 into an EHR during or at the end of the 6-month period, Care3 will provide this service on an ad hoc basis for $50

  • Care3 will use the DIRECT Protocol to move data from Care3 to an EHR

    • If PACE organization is a paid subscriber of Care3 mobile app, Care3 will provide this service on an ongoing basis for $5 per Participant per month

    • If PACE organization is a paid subscriber to the full Care3 platform, this service is included in subscription

  • NC PACE organization must agree to and sign the Care3 Business Associate Agreement at signup or manually for participation

  • Care3 will onboard all PACE organization staff who will be using the app

  • If desired, Care3 will provide in-person training on mobile app usage, features, and benefits

    • Training would include signup, on-boarding, text and media features, action messaging, and more
