Adding Medication and Symptom Logs Guide

In addition to the iPhone and Android apps, there is also an Care3 Action Planning (CAP) website you can access to easily add Reminders for symptoms, medications or other items to track in your logs!

Just follow these simple steps to create your personal Medications and/or Symptoms log (not shared with anyone else).

1 .  Go to http://www.care3.co and click the “Login” button in the upper right corner.

2 .  Sign in using the same email and password you use for the mobile app.

3 .  On the Dashboard, click the “New Action Plan” button.

4 .  On the “8 Steps to Success” page, you will complete Step 1, Step 2, and Step 6 for Medication and Symptom logs.

5 .  Click Step 1 and add only YOUR email address for client and family caregiver (the same email address you used for logging into the app and website). This tells the system you’re only sharing the logs with yourself. Click the “save & next” button in the upper right corner.

6 .  On the Step 2 page, add a Title for your log like, “Pain Log”. Then add a Start Date and an End Date for your log (start with 7 days).

7 . In the Problems field, choose the main Diagnosis. Add any other diagnoses and notes in the “Objectives” field. Click the “invite” button in the upper right. Click “ok”. Then click the “save & next” button next to the “invite” button.

8 . Skip Steps 3, 4, and 5 by clicking the “save & next” button on each Step.

9 .  On Step 6, you set your Medication and Symptom Reminders. Click the “add medication” button. Set Morning Reminders by choosing “Other” from the Medication dropdown menu and typing “Morning Meds”. Select the form and dosage. Add the frequency you’re supposed to take the medication. In the time fields that appear on the next line, select the time of day you want the Morning Reminder to be sent to you in the mobile app. Repeat these steps for Afternoon and Evening Reminders.

10 .  To set your Symptoms Tracking Log, click the “add symptom” button in the category (e.g. dysautonomia, cardiac, etc.). Select symptoms you want to track from each category from the dropdown menu. Be sure to select the Frequency and Times to schedule the Reminders. Click “save & next”.

11 .  Skip Step 7 (click “save & next”).

12 .  On Step 8, scroll down to view your logs. Click the “send” button in the upper right to send your Medication and Symptom Log Tracking Reminders to the mobile app. You will see your Reminders in the app the next day.
